Comprehensive Kidney Care Contracting (CKCC) is a Medicare value-based care model administered by the CMS Innovation Center. It is part of the Kidney Care Choices (KCC) Model, designed to improve outcomes for patients with advanced chronic kidney disease and end-stage renal disease through coordinated care, optimal dialysis starts, increased home dialysis, and transplantation.
