Northern Nephrology & Hypertension has cared for people with kidney disease and complex high blood pressure across northeastern New York since 2002. What began as the nephrology practice of Craig G. Hurwitz, MD, has grown into a multidisciplinary team that brings modern, evidence based kidney care to patients while keeping that care personal and close to home.

Our Story

From one practice to a kidney care team

The idea behind NNH has not changed since the first patient walked through the door: people living with kidney disease should be able to get real specialty care without leaving the region. What has changed is how many hands it now takes to do that well.

  1. 2002

    NNH begins

    Northern Nephrology & Hypertension was founded by Craig G. Hurwitz, MD, with a simple focus: high quality, personal kidney care close to home.

  2. Growing regionally

    Bringing nephrology to our communities

    NNH grew beyond Plattsburgh, helping patients throughout northeastern New York reach specialty kidney care without always having to travel to a distant medical center.

  3. Building a team

    More than one doctor

    Over time the practice became a multidisciplinary team of physicians, advanced practice providers, nurses, medical assistants, dialysis professionals, educators and administrative staff.

  4. Today

    Comprehensive kidney care

    NNH now cares for patients across the whole span of kidney disease, from early CKD and high blood pressure through advanced kidney disease, dialysis, home therapies, anemia management, infusion therapy, kidney stone prevention and other specialized nephrology care.

Our office is in Plattsburgh, and patients come to us from across northeastern New York. When someone needs care we cannot provide in our own building, we stay involved and work with the teams who can.

  • One office, in Plattsburgh. Everything we do runs out of 52 Tom Miller Road, Monday through Friday.
  • Patients from across the North Country. People travel to us from throughout northeastern New York for kidney care.
  • Hospitals in the region. Our work connects with CVPH Medical Center, Elizabethtown Community Hospital, Alice Hyde Medical Center and Adirondack Medical Center.
  • Teams beyond our walls. We coordinate with dialysis units, primary care practices, other specialists and transplant programs so care does not fall between them.

More Than an Office Visit

The long arc of kidney care

Kidney care is rarely one appointment and a prescription. It runs over years, and each stage asks something different of the team. Choose a stage to read more.

Protect kidney function and cardiovascular health

Blood pressure, diabetes, medicines that are hard on the kidneys, salt and the ordinary habits that decide how quickly kidney disease moves. A great deal of nephrology is quiet work done years before anything feels wrong.

Follow the numbers that matter

Kidney function, blood pressure, protein in the urine, anemia, electrolytes, bone and mineral levels. Trends over time tell us far more than any single result, which is why we watch them closely.

Evidence based therapy, matched to the person

Modern kidney therapies work, but not identically for everyone. Treatment is built around the diagnosis, the other conditions someone is living with, and what matters to them.

Understand the options early

When kidney disease advances there is a great deal to learn about transplantation, home therapies and dialysis. We start those conversations early, while there is still time to choose rather than react.

Stay with people for the long run

Care does not stop when dialysis or transplant begins. We keep working with patients and their families through treatment and through the years that follow.
